What's happening
Menopause isn't the end of anything — it's the start of a long phase of life that deserves real medicine and real attention.
Menopause is officially diagnosed 12 months after your last period. The average UK age is 51. Anything under 45 is 'early', under 40 is 'premature' (POI) and needs specialist input.
Symptoms often continue for years after that final period. Long-term health — bones, heart, brain, urogenital — also matters now.

Hot flushes and night sweats — can continue 5–10+ years post-menopause.
GSM
Vaginal dryness, painful sex, recurrent UTIs. Treat with vaginal oestrogen — safe for almost everyone.
Bone loss
Faster in the first 5 years post-meno. DEXA scan if at risk.
Brain fog + mood
Often improves on HRT. If not, get it reviewed.
Libido
Testosterone (off-licence in UK but recognised) can help when HRT alone doesn't.

Myths vs facts
Myth: You can only take HRT for 5 years.
Truth: There's no maximum duration. Annual review beats arbitrary stop dates.
Myth: Vaginal oestrogen is dangerous after breast cancer.
Truth: Increasingly used even then — with specialist input.
Myth: It's too late to start HRT in your 60s.
Truth: Best window is within 10 years of menopause, but later use is sometimes appropriate.
